Step 2: Required Documents for NEW FAMILIES (these will be submitted as part of the online application):
- $75.00 Registration Fee Per Student
- State Issued Birth Certificate
- Baptismal Record if Baptized Catholic at a parish other than St. Lawrence or MQOP
- If Transferring Schools – Current Report Card/transcript & MAP scores
- If Transferring Schools – Any Evaluations, IEP’s/504 Plans, or Accommodation Plans
- Additional Documentation Upon Request
